Behavioral Health Consultant

  Job Summary:

Operates as a behavioral health consultant (BHC) on the medical team utilizing the Primary Care Behavioral Health (PCBH) model of care. Acting as a generalist, the BHC sees patients of all ages and with wide ranging medical, behavioral health and behaviorally influenced conditions. As a fully integrated member of the primary care team, the BHC provides episodic behavioral health services and interventions to a high volume of patients.

Chronic Disease Coordinator

  Job Summary:

The Chronic Disease Coordinator will be responsible for outreaching for all Chronic Disease programs, registries, and all Chronic Disease Management activities at HOPE Clinic. The coordinator will help enroll patients in the chronic disease management programs such as remote patient monitoring for hypertension and diabetes and ensure that tracking and follow-up is conducted for these programs. The Chronic Disease Coordinator will also be responsible for ensuring the appropriate documentation and data entry for all patients in the Chronic Disease programs. This position will coordinate with HOPE Clinic staff to ensure program success. They will be responsible for remote patient monitoring documentation and tracking, patient education on chronic disease topics such as hypertension and diabetes, supply inventory, ordering, management and reporting work related to the Chronic Disease program.

COVID-19 Nurse

  Job Summary:

The COVID-19 Nurse will work within our clinical service function. Provide ongoing support to our clinical operations during the COVID-19 pandemic. Under broad supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Serve as nurse to oversee infection control, patient process and flow, and point of dispensing operations.

  Job Summary:

Examine and treat patients with a wide range of conditions and refer those with serious ailments to a specialist or appropriate facility. Typical job duties include taking patient histories, performing or requesting diagnostic tests, making diagnoses, prescribing treatment, administering vaccinations and performing follow-up examinations, as well as educating patients in disease prevention and health maintenance. Additional duties include preparing medical records, clinical reports and correspondences. Commit to train future family physicians to deliver quality health services to patients of diverse cultural, ethnic and economic backgrounds. Medical Billing and Collection Specialist

  Job Summary:

The medical billing and collection specialist is responsible for ensuring accurate billing, timely submission of electronic and/or paper claims, monitoring claim status, researching rejections and denials, documenting related account activities, posting adjustments and collections of Medicare, Medicaid, Medicaid Managed Care, and commercial insurance payers. The medical billing and collection specialist must possess critical thinking skills and understanding of Medicare, Medicaid eligibility requirements as well as commercial insurance payer payment methods to correctly record contractual adjustments base on payer contracts or government regulations. In addition, the medical billing and collection specialist must demonstrate proficiency with billing system to ensure all functionality is utilized for the utmost efficient processing of claims.

Patient Support Specialist

  Job Summary:

Facilitate patient flow by notifying the appropriate staff of the patient’s arrival. Provide support to medical records department managing patient records in compliance with HOPE Clinic guidelines.

Pediatric Medical Assistant

Clinical Rotations

Clinical Rotations’ Description: AAHC proudly opens our doors to host clinical rotations at HOPE Clinic in a variety of disciplines including family medicine, pediatrics, and OB/GYN. Please note that due to a high volume of applicants, only those who are matched with a preceptor will be contacted.

Effective August 2019 until further notice: HOPE Clinic is currently postponing its Nurse Practitioner (NP) clinical rotations program. Due to the current program changes at HOPE Clinic, there is limited capacity for NP students at this time. Please check back at a later time for any updates regarding the program
