REQUEST FOR QUALIFICATIONS

The Clinics Pathway Project Management Consultant will provide overall project management to ensure that timelines are maintained. Project management responsibilities include: the coordination and completion of projects on time within budget and within scope; oversee project logistics; set deadlines; assign responsibilities; monitor and summarize progress of project.

The Clinics Pathway Project Management Consultant will work directly with HOPE Clinic and the organization’s consultants to ensure deliverables fall within the applicable scope and budget. S/He will coordinate with and between departments to ensure all aspects of each project are compatible and will hire new talent as needed to fulfill client needs.

  Job Summary:

Operates as a behavioral health consultant (BHC) on the medical team utilizing the Primary Care Behavioral Health (PCBH) model of care. Acting as a generalist, the BHC sees patients of all ages and with wide ranging medical, behavioral health and behaviorally influenced conditions. As a fully integrated member of the primary care team, the BHC provides episodic behavioral health services and interventions to a high volume of patients.

Chronic Disease Coordinator

  Job Summary:

The Chronic Disease Coordinator will be responsible for outreaching for all Chronic Disease programs, registries, and all Chronic Disease Management activities at HOPE Clinic. The coordinator will help enroll patients in the chronic disease management programs such as remote patient monitoring for hypertension and diabetes and ensure that tracking and follow-up is conducted for these programs. The Chronic Disease Coordinator will also be responsible for ensuring the appropriate documentation and data entry for all patients in the Chronic Disease programs. This position will coordinate with HOPE Clinic staff to ensure program success. They will be responsible for remote patient monitoring documentation and tracking, patient education on chronic disease topics such as hypertension and diabetes, supply inventory, ordering, management and reporting work related to the Chronic Disease program.

Family Physician & Core Faculty Member of the Family Residency Program

  Job Summary:

Examine and treat patients with a wide range of conditions and refer those with serious ailments to a specialist or appropriate facility. Typical job duties include taking patient histories, performing or requesting diagnostic tests, making diagnoses, prescribing treatment, administering vaccinations and performing follow-up examinations, as well as educating patients in disease prevention and health maintenance. Additional duties include preparing medical records, clinical reports and correspondences. Commit to train future family physicians to deliver quality health services to patients of diverse cultural, ethnic and economic backgrounds. Clinical Rotations’ Description: AAHC proudly opens our doors to host clinical rotations at HOPE Clinic in a variety of disciplines including family medicine, pediatrics, and OB/GYN. Please note that due to a high volume of applicants, only those who are matched with a preceptor will be contacted.
