REQUEST FOR QUALIFICATIONS

The Clinics Pathway Project Management Consultant will provide overall project management to ensure that timelines are maintained. Project management responsibilities include: the coordination and completion of projects on time within budget and within scope; oversee project logistics; set deadlines; assign responsibilities; monitor and summarize progress of project.

The Clinics Pathway Project Management Consultant will work directly with HOPE Clinic and the organization’s consultants to ensure deliverables fall within the applicable scope and budget. S/He will coordinate with and between departments to ensure all aspects of each project are compatible and will hire new talent as needed to fulfill client needs.

Behavioral Health Consultant

Operates as a behavioral health consultant (BHC) on the medical team utilizing the Primary Care Behavioral Health (PCBH) model of care. Chronic Disease Coordinator

  Job Summary:

The Chronic Disease Coordinator will be responsible for outreaching for all Chronic Disease programs, registries, and all Chronic Disease Management activities at HOPE Clinic. The coordinator will help enroll patients in the chronic disease management programs such as remote patient monitoring for hypertension and diabetes and ensure that tracking and follow-up is conducted for these programs. The Chronic Disease Coordinator will also be responsible for ensuring the appropriate documentation and data entry for all patients in the Chronic Disease programs. This position will coordinate with HOPE Clinic staff to ensure program success. They will be responsible for remote patient monitoring documentation and tracking, patient education on chronic disease topics such as hypertension and diabetes, supply inventory, ordering, management and reporting work related to the Chronic Disease program.

Clinical Nurse

  Job Summary:

The Clinical Nurse will work within our clinical service functions. Provide ongoing support to our clinical operations during the COVID-19 pandemic including the operations for covid vaccine and testing. Under broad supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Serve as nurse to oversee infection control, patient process and flow, and point of dispensing operations for COVID-19 related responses. Assists with the Vaccine for Children (VFC) program oversight. This position will also be responsible for assisting with quality assurance work within the clinical departments at the HOPE.

The COVID Vaccine Medical Assistant Lead will be responsible for outreaching for COVID Vaccine registration and all COVID vaccine activities at HOPE Clinic. Answer calls regarding COVID Vaccine questions on scheduling. Ensure a large volume of patients are registered for our recurring COVID Vaccine events. The COVID Vaccine Medical Assistant Lead will also be responsible for ensuring the vaccine team conducts appropriate documentation and data entry for all patients receiving a COVID vaccine. This position will coordinate with HOPE Clinic staff and outside organizations to host successful vaccine events in and out of the clinic setting. They will be responsible for vaccine inventory, ordering, management, and reporting work related to COVID Vaccine. This job will also provide on COVID testing for patients as needed by clinic schedule.

The medical billing and collection specialist must possess critical thinking skills and understanding of Medicare, Medicaid eligibility requirements as well as commercial insurance payer payment methods to correctly record contractual adjustments base on payer contracts or government regulations. Optician

The Optician is responsible for helping patients achieve the right type of eyeglasses, lens shape or contact lenses in accordance with their eyesight or eye conditions. His/her duties include communicating with other eye care professionals to receive prescriptions and send them to Laboratory Technicians, providing patients with sample frames to fit their prescriptions and maintaining accurate customer data regarding their current or updated prescriptions. He/she will also be responsible for frames inventory and sales.

Optometric Technician

  Job Summary:

Optometric Technician will need to work in a fast-paced academic medical optometric setting located within a Federally Qualified Health Center and wants the opportunity for further growth in eye care. He/she will perform a wide variety of front desk tasks such as registering new patients, scheduling appointments, handling insurance forms, accepting payments, screening telephone calls, and verifying insurance for patients. The optometric technician would also be responsible for direct patient care in the clinical and optical setting and be comfortable assisting patients of all ages including infants and geriatric patients of various non-English ethnic backgrounds. We are open to training the right candidate if he/she shows eagerness and flexibility to learn new skills and possesses strong customer service experience.
