HOPE Health Pharmacy Is committed to serving the community by providing high quality Pharmaceutical services. Offering both 340B and non-340B medications, we aim to enhance access to vital medications and deliver cost savings to eligible patients who see all our outstanding HOPE Clinic providers.

Immunizations

Our Pharmacists play an important role in disease prevention by advocating for and administering immunizations.

Medication Therapy Management

Our Pharmacists perform and obtain necessary assessments of the patient’s health status by monitoring and evaluating the patient’s response to therapy, including safety and effectiveness.

Medication Synchronization

Pharmacy staff will review patients’ ongoing monthly medications and develop a plan to synchronize them so they can be picked up on a single day each month that is convenient for them.

Nutrition Counseling

Our Pharmacists work in collaboration with a Registered Dietician to support our patient’s needs with their medications and dieting based on disease state.

FTCA

As a Federally Qualified Health Center (FQHC), HOPE receives HHS funding and Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

HOPE Clinic is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
