HOPE Clinic is thrilled to partner with the University of Michigan in crafting new programs and resources dedicated to enhancing your experience at HOPE Clinic. Our goal is to design specialized healthcare programs, educational tools, and easily accessible support systems that cater to the specific needs of young people between the ages of 10-24. As you get older, it’s important to learn all you can about your health, any chronic illnesses you have, and become self-reliant when it comes to managing your health care. For us to achieve this, we would like to hear from you! Please take a moment of your time to complete this survey so we can continue to make improvements and provide the best health care home for you!
The survey asks how “today’s visit” went. Please answer the questions from your last visit. Your answers are anonymous and we would really like to hear from the patient who was scheduled for the visit.
