Bite of HOPE is a collaborative initiative with Cities Changing Diabetes focused on fostering healthier communities in Houston, specifically in underserved neighborhoods with limited access to nutritious food. This innovative program integrates culinary practices into clinical care, reshaping community perspectives and interactions with food. By making healthy eating more accessible, Bite of HOPE aims to combat the rising prevalence of lifestyle-related chronic diseases, alleviating their economic burden and significantly improving health outcomes.
Designed to transform the future of communities affected by obesity, diabetes, high blood pressure, and cardiovascular disease in Houston, Bite of HOPE harnesses the transformative power of food. By teaching communities to prepare simple, cost-effective, and delicious meals, individuals can better manage their health conditions, leading to healthier and happier lives.

Programs and Initiatives

In addition to offering extensive community health programs and in-depth health literacy education, HOPE Clinic provides crucial support in chronic disease management, women’s health, infectious disease control, and behavioral health literacy. By seamlessly integrating these services, HOPE Clinic significantly enhances overall well-being and fortifies resilience within the communities it serves. Emphasizing a Health Equity approach, the clinic addresses both medical and socio-environmental needs, ensuring a comprehensive response to the diverse challenges faced by its patients.

Food Rx and FoodRx for Health
In partnership with the Houston Food Bank, the FoodRx initiative supports food-insecure patients battling uncontrolled diabetes. Through this powerful collaboration, patients receive fresh, nutritious produce sourced from local food pantries, addressing their dietary needs and encouraging healthier eating habits. This essential resource is complemented by the Bite of HOPE Chef, who provides cooking demonstrations and teaches recipes using the produce patients pick up that day. By increasing access to nutritious food, individuals can better manage their health, reduce the financial stress of household grocery costs, and achieve improved health outcomes and decreased food insecurity, fostering a path towards better health and a brighter future.

Walk with HOPE
HOPE Clinic’s innovative walking initiative invites patients, their families, guardians, and the broader community to participate in group walks. This program not only promotes physical activity but also provides a unique opportunity for families and community members to connect with healthcare providers outside the exam room. By fostering open communication and building strong relationships, this initiative encourages healthy habits and strengthens the bond between patients, their families, healthcare providers, and the community.
Asian American Attaining Awareness
The Asian American Attaining Awareness Program (AAAA) is a collaborative effort led by the Asian American Health Coalition - HOPE Clinic and funded by the Hogg Foundation for Mental Health. This initiative focuses on raising awareness about mental health, enhancing resilience, and improving overall well-being within the Asian American and Pacific Islander (AAPI) community in the Houston Metropolitan Statistical Area (MSA). AAAA aims to empower and uplift the AAPI community by addressing mental health needs and promoting a healthier and more resilient future.

Through community assessments, interventions, capacity building, awareness campaigns, and youth initiatives, AAAA has fostered mental health awareness, enhanced resiliency, and improved access to behavioral health services for the Asian community. With initiatives like Mental Health First Aid training, AAAA actively raises awareness, participates in events, and empowers youth to advocate for better behavioral health access.

Maternal & Child Health Programs
HOPE Clinic’s maternal and child program is dedicated to improving maternal mortality rates and infant birth outcomes. This initiative ensures comprehensive care for women before, during, and after pregnancy.
The program adopts a holistic approach, addressing immediate health needs while also tackling the social and environmental factors that influence well-being. We offer innovative services such as CenteringPregnancy® group prenatal care, which fosters a supportive community and personalized care for expecting mothers. Additionally, we host baby showers to celebrate new families and provide essential resources.
With the support of community health advocates, HOPE Clinic connects women and their families to preventative measures and available resources, fostering education and access within the community. This initiative is designed to support and uplift maternal and child health through a comprehensive, community-focused approach.

Latent Tuberculosis (LTBI) and Infection Disease Program
HOPE Clinic's integrated LTBI and Infectious Disease Program addresses latent tuberculosis infection (LTBI) and other infectious diseases. The LTBI program provides screening, education, and treatment with the 3HP modality, available in-clinic or via video, overcoming challenges like medication shortages with unwavering commitment.
The Infectious Disease Program focuses on HIV/AIDS education, treatment options, and preventive measures, with navigators facilitating access to care, supporting adherence, and coordinating comprehensive plans. The program also offers emotional support, risk reduction, and crisis intervention for diverse communities, including LGBTQ+ individuals and marginalized groups. Together, these programs enhance community health and well-being.
Hypertension Remote Patient Monitoring
HOPE Clinic offers state-of-the-art medical patient monitoring utilizing Bluetooth-enabled blood pressure monitoring devices for adult patients. Qualified patients with uncontrolled high blood pressure can receive an at-home monitor, granting both patient and healthcare provider electronic access to blood pressure readings. This innovative approach facilitates proactive communication between provider and patient, allowing for timely adjustments to medications and improved blood pressure management. Patients are empowered to actively monitor their results, taking proactive steps to enhance their lifestyle and overall well-being. Recognizing the importance of remote patient monitoring tools, HOPE Clinic actively engages in national and statewide initiatives aimed at optimizing the effectiveness of such technologies in healthcare settings.


MD Anderson CPRIT CoHEAL Liver Study
HOPE Clinic has partnered with MD Anderson Cancer Center, with funding from the Cancer Prevention Research Institute of Texas, to conduct the Assessment, Intervention, and Measurement (AIM) study. This collaboration aims to refine liver cancer screening methods.
The study has two phases: AIM I involves a comprehensive baseline evaluation, with nearly 1,000 patients enrolled and nearing completion. AIM II focuses on lifestyle interventions to enhance liver health in patients identified with risk factors for liver disease. Positive test results related to Hepatitis or liver disease trigger follow-up care and support.
Through this impactful research partnership, HOPE Clinic underscores its commitment to advancing liver health and contributing to crucial advancements in this field.
