As a patient, you have certain rights and responsibilities. We recognize that a respectful relationship between the healthcare provider and the patient is the foundation of proper medical care.

HOPE Clinic Policy

HOPE Clinic seeks to provide quality health services consistent with community standards and to utilize its resources wisely in order to enhance its mission. We want to be your primary medical home. As your “medical home” HOPE Clinic is your partner in improving your health outcomes through the following:

  • Annual wellness check so that health issues may be identified early
  • Enhanced access and communication through eClincal Works (eCW) patient portal
  • Streamlined coordination of care with providers

Patients have the right to:

  • Receive humane care and treatment, with respect and consideration.
  • Privacy and confidentially when seeking or receiving care except for life threatening conditions or situations.
  • Confidentiality of your health records.
  • Be informed and to exercise the option to refuse to participate in any research aspect of your care without
    compromising access to medical care and treatment.
  • Receive accurate information concerning diagnosis, treatment, risk involved, and prognosis of an illness or health
    related condition.
  • Ask about reasonable alternatives to care.
  • A second professional opinion regarding one’s health care and treatment.
  • Accessible information regarding the scope and availability of services.
  • Be informed about any legal reporting requirements regarding any aspect of screening or care.
  • File patient grievances with HOPE Clinic management.

Patients have the responsibility to:

  • Provide complete information about one’s illness/problem, to enable proper evaluation and treatment.
  • Ask questions so that an understanding of the condition or problem is ensured.
  • Supply proper documentations for eligibility screenings
  • Communicate with care coordination staff for timely referrals
  • Show respect to health personnel, HOPE staff, and other patients.
  • Timely reschedule/cancel an appointment so that another person may be given that time slot.
  • Pay bills or file health claims in a timely matter.
  • Use prescription or medical devices for oneself only.
  • Inform the practitioner(s) if one’s condition worsens or an expected reaction occurs from a medication.
  • Notify HOPE Clinic of any changes in telephone, address, or insurance as soon as possible.

Patients may be discharged from the practice immediately if they are deemed to be abusive or threatening to the staff. Patients who are perceived to be difficult, with respect to recurrent hostile behavior, inappropriate use of HC services, excessive non-compliance, inappropriate use of controlled substances, or other patterns of behavior that represent excessive lack of respect or responsibility on the part of the patient, may be referred to the Medical Director for review by the Patient Safety Committee.

FTCA

As a Federally Qualified Health Center (FQHC), HOPE receives HHS funding and Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

HOPE Clinic is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
