Walk-in patients are welcomed, but we encourage you to call and schedule an appointment:

Please bring with you:

• Two forms of I.D.
• Proof of income (W-2 or Pay stub)
• Insurance card if applicable
• Any medications you take including vitamins
• Immunization record if applicable
• Child's birth certificate if applicable
• Patient forms (please refer to "Services" for downloads the forms)
