How much do I need to pay?
HOPE Clinic charges a minimum of $20.00 per visit depending on the income of the patient. Discounted labs are extra.
We also accept private insurance, Medicare and Medicaid.
What do I need to bring with me on my first visit?
Please bring with you:
- Two forms of I.D.
- Proof of income (W-2 or Pay stub)
- Insurance card if applicable
- Any medications you take including vitamins
- Immunization record if applicable
- Child's birth certificate if applicable
- Patient forms
